Tumour differentiation Necrosis Mitotic count (n per 10 high power fields)
1. Well 0: Absent 1: n < 10
2. Moderate 1: <50% 2: 10–19
3. Poor (anaplastic) 2: ≥50% 3: n ≥ 20
  1. The sum of the scores of the three criteria determines the grade of malignancy. Grade 1 = 2 or 3; Grade 2 = 4 or 5; Grade 3 = 6
