Treatment schedule. Chemotherapy (CT): ≥ 18 and < 70 years of age: Day 1: doxorubicin 60 mg/m2, 4-h infusion (IV); Day 1, 2 and 3: ifosfamide 2 g/m2/day as 2-h infusion, dose per cycle 6 g/m2 [with an equal dose of 2-mercaptoethane sulphonate sodium (MESNA)]. ≥ 70 and ≤ 75 years: doxorubicin 50 mg/m2 and ifosfamide 5 g/m2 (given as above). Granulocyte colony-stimulating factor was given routinely after each cycle
